Through e-mail, text or VM, give me a brief summary of your history, symptoms, and if you have a diagnosis already. Please do not share this information on FB.
I will ask you to sign up on Practice Better, the electronic medical record (EMR) I use for my practice. Through that, I will send you the intake forms. This system is HIPAA compliant. These forms can be completed on line and submitted directly to your record in Practice Better. If you have records of lab tests, imaging studies or letters from other providers, you can upload them directly to your chart in Practice Better. We can also communicate with a messaging program through this app (similar to MyChart or other EMR portals).
Once you return the questionnaires and records through the app, I will review your information and contact you to make an appointment. I do require 1/2 payment in order to make the appointment. Please see the payment pages for rates and explanation of payments. Since I am just starting out, I have virtually NO waitlist!

Please Note: At this time, for telemedicine visits I can only see individuals with a permanent residence in Illinois because of licensure requirements for telemedicine. I am looking into the logistics of seeing people who are out of state. I can see anyone who wants to travel to my office for an in-person visit, but I may not be able to follow you long term if you live out of state. If you are simply looking for an official hEDS/HSD diagnosis, that can be done at the initial visit.
