Payment Details

After you contact me, I will send you a link to Practice Better an electronic medical record (EMR) system. Through that, I will send you required intake forms. This will help me determine our starting point and understand your concerns. Once you upload the forms I will take some time to review – then contact you to set up an appointment.

I require 1/2 payment in order to set up the appointment. The other 1/2 is expected at the time of the visit. Rationale: Since I am cash based, this is the only way I know for sure I will get payment for my services.

If the office visit is cancelled and rescheduled, there is no fee. However, if the office visit is cancelled and rescheduled 3 times, I will no longer be able to reschedule you (and will charge a fee according to statements below). Rationale: It takes time to set up appointments and if you have cancelled 3 times, I am concerned about your commitment to my program.

If the office visit is cancelled and not rescheduled, I will return the down payment minus $50. Rationale: This is a fee for the office set up as well as review of the records and questionnaire in preparation for the visit.

If the office visit is cancelled within 24 hours of visit and not rescheduled, I will return the down payment minus $100. Rationale: As above and if you cancel with such short notice, I will not be able to fill that space with another patient and I will lose money.
