Cash
1/2 must be paid by another form when setting up appointment. The remainder can be paid in cash if seen in person
Check
If paid by check, you must pay 1/2 two weeks in advance of appointment being scheduled and the remaining 1/2 two weeks in advance of actual appointment. This gives an opportunity to make sure the check has sufficient funds.
