Comprehensive Functional Program

New/initial visit: $300

Includes Physical Exam, Review of Questionnaires, Complete Personal, Family, and Life History.

Includes 3 F/U questions (Includes multiple calls or MyChart/email responses for each question until question is answered). After 3 questions, $25-50 charged per response. By that time, we may be on the next visit anyway.

Appointment will be considered a new visit if I have not seen you for >2 years.

Return/Follow up Visit: $150

Each visit will focus on a different Modifiable Lifestyle Factor (MLFs) – see Clinic Services Page.

Each return visit includes 2 questions (includes multiple telephone/email responses for each question). After that, each different question is $25-50 charged depending on time spent and resources used. By that time, we may be on the next visit anyway.

Return visits are any return within 2 years of the initial visit until Program is Complete

Full Functional Program: $1000 (saves $200) if paid in full before first visit

Full Functional Program includes Initial visit, plus 5 Modifiable Lifestyle Factors – and Mental/Spiritual/Emotional Assessment and wrap up session (Includes 7 total visits)

All questions and responses are included at no limit and no extra charge.

Subsequent Follow-up Visit (after completing Entire Program): $100

Physicals (Work, School, Sports, Camp)

Sports/Camp Physical: $60 (ask about discounts for McHenry Deanery)

School Physical (Pre-K, K, 2nd, 6th, 9th Grade): $75 (ask about discounts)

Work Physical: $75

Physicals do not include labs or immunizations. If you need labs, I have an account with Quest, but I can send you with orders to a lab of your choice. If your child needs immunizations, I will send you to the department of Public Health in your county. My fee does not include the cost of Labs or Immunizations.

Lifestyle Optimization Program

Each Session is $100. Includes 6 months of follow-up calls/messages for each section. 

Physical Exam associated with any visit is an additional $50. 

Adding laboratory testing if warranted is an additional $25 per visit.

Full Lifestyle Optimization program: $500 (saves $200) if paid in full before first visit.

The Full Lifestyle Optimization Program includes 7 visits and unlimited follow-up calls for up to 6 months after the last session. There will be a one time fee of $50 for all physical exams and a one time fee of $50 for all laboratory and imaging orders.

Each visit Includes review of Questionnaire with Assessment and Plan regarding each of the following Topics:

You may start and stop at any section depending on needs. For example, a person may feel that they need help with nutrition and stress reduction, but not sleep. No matter how many sections are completed, I recommend a wrap up session to discuss the results of the plan. 

Connective Tissue Disorder Diagnosis

Fee: $150. (Genetic testing is a separate fee based on which lab is used.)

A request to provide additional letters of medical necessity (beyond my initial request), peer to peer review, appeal, and/or prior authorization can be done at a rate of $100/hour.  Click here for an understanding of why I must charge for my time for this.

Includes Basic Connective Tissue Disease Assessment (History and Physical Exam) and Ordering Genetic Testing (if warranted) 

I use the following lab companies for genetics: Ambry, Invitae, or Quest (managed through Blueprint testing). These labs may work with your insurance and if the insurance denies payment, you may be able to pay a reduced fee out of pocket. All of this will be negotiated directly with each company.

A licensed provider such as a doctor, a nurse practitioner, or a genetic counselor is typically required to order genetic testing.
