There are many situations where a person needs a physical exam. These exams are typically done through the primary provider, but there may be times when you have to get the exam done and your primary does not have any openings.
School Physical
Most states require a comprehensive history and physical exam for entry into school. In Illinois, this is required for all Pre-K, Kindergarten, 6th Grade, and 9th Grade. Most colleges will also require a physical.
These grade levels also require updated immunizations. I am not able to complete the immunizations, but I can refer to a local county health department for the necessary shots. We can discuss your desire to avoid immunizations or to spread out immunizations over time. While I personally and professional believe in immunizations if they prevent the disease, I know that this is often a family decision.
Sports Physical
According to the Illinois High School Association and Illinois Elementary School Association all students grade 5 through 12 who participate in inter scholastic sports are required to have a physical exam to participate in sports. Please ask about discounts for McHenry Deanery.
Camp Physical
Some camps may require a physical exam to attend.
Work/Travel Physical
If your place of employment or organization you are traveling with requires a physical exam.

Please remember that if you or your child has a long standing illness and is being treated by a specialist, you may need additional paperwork from them clearing you or your child for participation in Sports, Camp, or Travel. Some examples may be but not limited to cardiac disease, or seizures.
